Moving from children to adult services
Transition is the word often used to describe the process when you move from being a child to an adult. It covers the period between ages 14 and 25. Throughout childhood, anyone with complex disabilities will have had support from health, social care and education services. As you get older, you may need similar support from adult services to meet your changing needs. At Advocating Care Services we can help you to:
- Move to adult healthcare services.
- Move to adult social care services.
- Go from school to college.
- Get a job or start training.
- Manage housing and benefits.
- Prepare generally for adult life, including managing money, transport, a social life, friendships and relationships.
Advocating Care Services offers a wide range of supports to assist life stage transitions. The idea is to make this transition phase as smooth as possible without disrupting the comfort of the participant. We work with the participant to enable a friction-free move and encourage the participant to settle in a new setting. We offer disability care services to help our NDIS participants through crucial life stage transitions. We aim to offer you the best assistance possible, no matter the transition phase you are going through.
